Jinzhen oral liquid (JZOL) is extensively employed within China's healthcare system. Nevertheless, the tissue-specific distribution of this material, essential to studies on the efficacy of these substances, has yet to be documented. Mice were used to investigate the chemical components, including prototypes and metabolites, of the substance, and to assess its distribution within different tissues in both healthy and diseased specimens. Characterization revealed several constituents, including 55 identified in JZOL, 11 absorbed prototypes, and 6 metabolites present in plasma and tissue samples. Demethylation, dehydration, and acetylation were components of the metabolic pathways. A sensitive, accurate, and reliable quantitative approach was created for determining the spatial distribution of components within the tissue. The seven components, following JZOL's administration, experienced rapid distribution across diverse tissues, predominantly accumulating in the small intestine, with reduced presence in the lung, liver, and kidney. Healthy mice showed superior absorption of baicalin, wogonoside, rhein, glycyrrhizic acid, and liquiritin apioside relative to those in influenza mice, while the latter demonstrated a slower elimination rate. Instances of pyomyositis, although infrequent, are typically connected to factors such as poorly managed diabetes, a history of trauma, and a weakened immune response. An elderly woman, afflicted with diabetes mellitus for two decades, and whose breast cancer, following a modified radical mastectomy and chemotherapy 28 years ago, is now in remission, is the subject of our discussion. The patient exhibited a gradual swelling of the shoulder accompanied by significant pain. The examination concluded with the diagnosis of pyomyositis, requiring the performance of debridement surgery. An unforeseen diagnosis of primary biliary cholangitis (PBC) was made during the patient's hospital stay, in addition to the documented poor management of blood sugar levels.
